Insurance and Self Pay Coverage

Consultations are fully covered with participating insurances.

If your insurance is not covered there are self -pay options: At your request a super bill will be provided to you to submit to your insurance company for reimbursement. Reimbursement is not guaranteed by the insurance company. We recommend contacting your insurance company prior to receiving services to determine coverage and reimbursement policies/process. Check with your insurance company to see if they require pre authorization. Out of network? Request “in for out” of network coverage or “gap exception” from the insurance company. This allows your insurance company to cover the services as “in-network,” to fill the gap they have in their own network.

Cancelation Policy

We aim to be flexible and accommodating with scheduling and understand that cancelation is often inevitable. However canceling or rescheduling needs to be done 24 hours prior to appointment time. If you fail to cancel within that time frame you will be charged a $50 fee.

Travel Fee

A travel fee of $20 will be added for any visit more than 30 mins drive from our location. You will be notified of this fee at the time of booking.
